TLC is sharing more stories of emotional weight-loss journeys in 2025. For the past 12 years, My 600-lb Life has profiled morbidly obese people who’ve turned to Dr. Younan Nowzaradan in an effort to lose weight and change their lives. A new season of the reality series debuts on New Year’s Day, just as many viewers are kick-starting their own weight-loss resolutions.

My 600-lb Life Season 13 premieres Wednesday, Jan. 1 at 8 p.m. ET/PT on TLC. Episodes stream the next day on Max. The new season will follow seven new patients in two-hour episodes. They’ll navigate the highs and lows of extreme weight loss, with the support – and tough love – of Dr. Now.  

The Houston-based surgeon is dedicated to helping people lose weight and live healthier lives, even when that seems like a nearly impossible task.  

“It’s a daily challenge to work with some patients that can be self-destructive,” Dr. Now told People in 2017. “My job is not to get aggravated, but to find a way to motivate them to work hard to get to their goals. There are times where I think it’s necessary for some tough love and I have to be stern with them, so I show some of my concern and frustration.”

Each My 600-lb Life patient will have to make extreme diet and lifestyle changes, including high-risk bariatric surgery, to achieve potentially life-saving results. This season will highlight the stories of a pastor who is eager to lose weight so he can return to the pulpit, a woman who must overcome her fear of leaving the house if she wants a better life, a former first responder who has vowed to make a major change, an overprotective son whose love for his mom is preventing him from getting the help he needs, and a husband who is desperate for his wife to get healthy. 

An EMT tries to break a harmful cycle on this season of ‘My 600-lb Life’ 

In the My 600-lb Life Season 13 premiere, viewers will meet Jonathan. 

Once, Jonathan had his dream job, saving other people’s lives through his work as an EMT. Now, he rarely leaves his room. His wife must work full-time to pay their bills. If he wants to stay married and keep his life, he’ll need to break his unhealthy cycle of eating. 

Episode 2 features Krystal A. She’s realized that her weight is getting in the way of living the life she wants. She has a loving husband who supports her unconditionally. But if they want to have a happy life together, she knows she needs to get weight-loss surgery.
